The Registered Nurse (RN), Mendota Health Center role is onsite in Mendota, IL, with compensation of $28–$31 per hour based on experience and qualifications. Community Health Partnership of Illinois provides primary medical, dental, and behavioral health services to migrant farmworkers and other medically underserved populations in Illinois. The RN provides patient-centered nursing care in collaboration with providers and the healthcare team. Through assessment, treatment, education, documentation, and care coordination, the role supports access to care and improved community health outcomes.

What you'll do

• Assess patients and monitor health status
• obtain and document vital signs, medical histories, and clinical information
• provide nursing care under established protocols and provider direction
• administer ordered medications, immunizations, and treatments
• assist providers with examinations, procedures, and follow-up
• educate patients on medications, treatment plans, preventive care, and chronic disease management
• coordinate care and communicate with the healthcare team
• maintain confidential electronic health records
• support infection control, quality, and patient safety
• connect patients with follow-up care and community resources.
