What you'll do

• Accompany individuals to community activities and appointments
• encourage hobbies, fitness, and social participation
• support friendships and community connections
• help build confidence and independence
• assist with daily activity planning, routines, organization, time management, goal setting, transportation, and daily living skills as needed
• complete service documentation
• communicate with supervisors, families, and support team members
• follow individualized support plans and agency policies
• attend required training and meetings.
