The physician provides comprehensive primary care to patients of all ages, managing acute and chronic conditions and delivering preventive care. The role supports continuity of care, coordination with specialists and care teams, and quality improvement initiatives across the network.

What you'll do

• Provide outpatient primary care to patients of all ages
• diagnose and manage acute and chronic conditions
• conduct preventive visits, annual wellness examinations, and screenings
• coordinate care with specialists and ancillary services
• document care in EPIC EMR
• participate in quality improvement and value-based care initiatives
• collaborate with advanced practice providers and multidisciplinary teams
• build long-term patient relationships.
