What you'll do

• Monitor residents’ changing needs and coordinate timely interventions
• conduct pre-admission assessments
• perform compliance audits
• coordinate care plans and conferences
• oversee MDS compliance and admission documentation
• support staff during clinical emergencies
• communicate with nursing staff, physicians, pharmacists, residents, and families
• interview, train, and mentor frontline staff
• participate in the on-call rotation as needed.
