What you'll do

• Provide skilled nursing services within professional practice
• document measurements and symptoms and provide medication education or adherence support as appropriate
• collaborate on plans of care
• monitor CCBHC outcome measures and treatment-plan progress
• coordinate routine, urgent, and crisis care across medical, behavioral, dental, pharmacy, and specialty services
• arrange appointments and follow-up
• support prescription authorizations and adherence
• obtain provider information and facilitate referrals
• complete clinical documentation.

Why this role

Requires an RN or LPN license to practice in Minnesota. CPR/AED/First Aid certification is required or must be obtained. Travel to client or organization locations throughout southeast Minnesota is required as needed.
