What you'll do

• Complete behavioral health, psychosocial, functional, and social-needs assessments
• develop care plans with patients and the care team
• provide counseling, supportive therapy, motivational interviewing, behavioral activation, and crisis intervention within scope
• assess safety concerns and escalate when needed
• coordinate with providers and community organizations
• support transitions of care
• connect patients to resources
• educate patients and families
• participate in case conferences
• complete clinical documentation.
