What you'll do

• Manage Referral Coordinators and ensure referrals and appointments are processed, tracked, and completed promptly
• optimize care coordination, patient education, scheduling, and loop closure
• address insurance verification and eligibility issues
• develop referral pathways and maintain partner relationships
• lead special projects
• communicate workflow changes
• monitor KPIs and staffing
• coach, supervise, and onboard staff
• maintain referral documentation and accreditation competencies
• assist with coordinator duties as needed.
