What you'll do

• Use market research, referrals, networking, and referral-source relationships to support patient referrals and admissions
• coordinate assessment, planning, facilitation, care coordination, evaluation, and advocacy for workers’ compensation patients
• assist with clinical assessments
• educate referral partners about services and long-term acute care placements
• develop client partnerships and maintain referral relationships.
