What you'll do

• Obtain and record vital signs
• collect blood and urine samples as requested
• reconcile home medications
• complete triage and nursing assessments
• determine care pathways and connect clients with the next provider
• monitor physical and behavioral health
• communicate with the clinical team
• conduct violence-risk screening
• review prescription monitoring information
• provide medication education
• facilitate telehealth evaluations
• call in prescriptions and refills as directed
• document care
• follow agency policies and certification standards.
