What you'll do

• Plan, develop, organize, implement, and evaluate social services programs
• build relationships with residents and provide counseling or assistance for psychosocial needs
• complete social histories and assessments
• participate in discharge planning and social care plans
• involve residents and families in program planning
• coordinate referrals and resources
• work with agencies on residents’ behalf
• coordinate admissions, transfers, and discharges
• maintain progress notes and records
• process Medicare denial letters
• support staff training and facility orientation
• maintain confidentiality.
