What you'll do

• Problem identification, psychosocial assessment, financial counseling/referral, accessing community resources, placement for care, guiding members through health-related legal processes, and consultation to other healthcare professionals.
• Develops and monitors care plans, advocates for organizational and community resources, and ensures consistent documentation.
• Facilitates application processes for Medicaid, Medicare, and Disability, and provides supportive counseling to members and families.
