The Registered Nurse (RN) role is onsite in Florence, KY, with a salary of $40.55–$48.35 per hour. Senior CommUnity Care of Northern Kentucky PACE provides personalized healthcare and supportive services to adults age 55 and older so they can live safely and independently in their communities. The nurse provides general and skilled nursing care, implements care plans and primary-care orders, assesses and responds to participant needs, and coordinates with caregivers, physicians, and the interdisciplinary team. The work supports participants’ health and continued community living through clinic and in-home care.

What you'll do

• Provide general and skilled nursing care and implement care plans and physician orders
• assess and reassess participants as directed
• observe, document, and report conditions and treatment responses
• triage complaints and care needs
• coordinate with the interdisciplinary team and outside providers
• educate participants, families, and staff
• maintain records and confidentiality
• participate in the nursing on-call rotation for after-hours and weekend assistance as needed.
