What you'll do

• Complete initial and ongoing assessments of physical, emotional, and psychosocial needs
• develop and update care plans with patients and families
• provide skilled nursing, pain and symptom management, medication administration, and comfort measures
• educate patients and caregivers
• coordinate care and participate in interdisciplinary group meetings.
