What you'll do

• Provide care to hospice patients and families
• assess physical, emotional, and psychosocial needs
• develop and implement individualized care plans with the interdisciplinary team
• administer prescribed medications and treatments
• monitor responses and adjust care plans as needed
• educate and support patients and families
• coordinate care with healthcare professionals
• document care accurately and promptly.
