What you'll do

• Observe and document patients’ physical, mental, and emotional needs
• consult with the Hospice Nurse about patient status and symptom management
• deliver prescribed physical and psychological care under supervision and team approval
• monitor and document patient and family responses
• communicate with families and health professionals
• participate in interdisciplinary meetings
• maintain home-care supplies
• collect requested documentation
• assist with clinical billing as needed
• participate in orientation of new hospice aides as assigned.
