The Home Health Registered Nurse (RN) role is an onsite field position serving Dublin and the listed Area 3 and Area 4 communities in California. Compensation is $90,000–$160,000 per year for the full-time schedule. CVHCare provides home health clinical services. Paid training is onsite in San Ramon, CA, Monday through Friday, 9:00 a.m.–5:30 p.m. The nurse monitors patients’ health, develops individualized plans of care with primary care physicians, and manages cases independently. Home visits, patient education, and specialized care support patients’ health needs in their homes.

What you'll do

• Monitor patients’ overall health
• partner with primary care physicians to develop individualized plans of care
• function as a case manager
• provide patient education about disease management
• deliver wound therapy, post-surgical care management, and ostomy care management
• plan patient visits and schedules.

Why this role

Full-time schedule is 24 RVE per week; part-time option is 18 RVE per week. Must be available to work up to two weekends per month. Non-traditional schedules may be available. Requirements include an active California RN license, at least one year of RN experience, reliable transportation, and current CPR or BLS card.
