What you'll do

• Assess incoming patients and identify care needs
• collaborate with physicians and the interdisciplinary team on care plans
• explain admissions processes and services to patients and families
• complete admission paperwork and obtain required records and forms
• coordinate assessments and services with hospice disciplines
• administer prescribed medications and treatments
• monitor patient condition and report changes
• provide emotional support
• follow regulatory requirements
• participate in team meetings.
