What you'll do

• Assess needs and coordinate individualized care plans with the interdisciplinary team
• monitor progress and communicate updates to clients, families, guardians, and providers
• coordinate care conferences, utilization reviews, and payer authorizations
• support discharge planning, referrals, and transitions of care
• maintain accurate documentation and track quality measures.

Why this role

At least three years of case management experience required, preferably in rehabilitation or a related healthcare setting; bachelor’s degree in a related field preferred; brain injury or neurological condition experience and CCM or ACM certification preferred.
