Assesses and triages patient clinical information as shared by the referring provider and utilizes process flows, emergent and/or specialty algorithms and admission guidelines to assign and transport patients to the right facility with the most appropriate level of services. Facilitates conference calls between the referring provider and the accepting physician and communicate identified concerns
