Transition Therapist

The Transition Therapist role is onsite in Tinley Park, IL. Compensation is $65,000 per year. The therapist supports Envision Unlimited’s interdisciplinary services for people with mental illness or physical disabilities transitioning from long-term care to community living. Envision Unlimited provides services to individuals with disabilities across Chicagoland and Central Illinois. The role provides clinical support to help clients address fear or anxiety about moving into the community and prepare for a successful transition. Assessments, counseling, crisis planning, and coordination with clients’ treatment teams support continuity of care and community placement.

What you'll do

• Conduct assessments
• provide onsite supportive counseling to clients residing in nursing homes and SMRFs
• use motivational interviewing and other techniques to support decisions and long-term goals
• help clients identify crisis triggers and contingency plans for after discharge
• maintain client information in the mental health electronic care system
• complete clinical documentation and service reporting
• keep families, guardians, and treatment team members informed of progress and critical incidents
• coordinate with outside agencies
• provide and document crisis interventions as needed within 24 hours of an incident.

Why this role
