What you'll do

• Complete Minimum Data Set assessments accurately and on time
• coordinate the Resident Assessment Instrument process
• lead interdisciplinary care plan meetings
• review medical records and clinical documentation
• code MDS items to reflect resident acuity and support appropriate PDPM classification
• and track Case Mix Index and quality measure data.
