What you'll do

• Assess patients and families, develop and revise care plans, provide nursing care and prescribed treatments, respond to triage calls, make visits for changing conditions, and complete weekend admissions.
• Maintain clinical notes, communicate with physicians and community health professionals, educate patients and caregivers, and coordinate with the hospice interdisciplinary group.
