The role supports continuity of care for a defined patient panel through examinations, treatment planning, medication management, documentation, and coordination with clinic staff and referring clinicians.

What you'll do

• Provide primary care within scope of practice and privileging
• deliver preventive, diagnostic, and treatment services for acute and chronic conditions
• provide women's health services to a defined patient panel
• take histories, perform examinations, order tests, and formulate treatment plans
• manage medications under VA policies
• refer patients to specialty or hospital care when appropriate
• document care in CPRS
• consult with referring clinicians and provide patient education and counseling.
