What you'll do

• Identify residents’ social and emotional needs
• develop and maintain individualized care plans
• participate in admission, re-entry, and transfer planning
• contribute to interdisciplinary care plan meetings
• coordinate resident financial documentation with the finance team
• arrange discharge equipment or home care
• assess prospective residents for facility placement.
