What you'll do

• Deliver comprehensive diabetes care and education integrating clinical and self-management needs
• collaborate with the inter-professional team
• support and advocate for people affected by diabetes
• promote self-management to meet individualized goals and optimize health outcomes
• obtain CDCES certification when certification requirements are met if not already certified.
