Registered Nurse (RN) Case Manager

The Registered Nurse (RN) Case Manager role is onsite in Lansing, MI, with a Monday–Friday schedule from 8:00 a.m. to 5:00 p.m., 40 hours per week. Compensation is $31.28 per hour. The role is part of the Supports Coordinator Team serving older adults and people with disabilities who are at risk of entering nursing homes. The RN works with participants and a social worker to assess needs, develop person-centered care plans, coordinate and monitor services, and advocate for access to supports. This work helps participants pursue alternatives to institutional care while maintaining program requirements and individual goals.

What you'll do

• Assess participants' physical and mental health, functional abilities, support systems, environment, and resources
• develop person-centered care plans
• coordinate, monitor, and follow up on services
• support Medicaid Waiver eligibility and documentation
• advocate for participant services and choice
• maintain a shared caseload
• and cover scheduled 24-hour on-call services. Requires a current Michigan RN license, one year of nursing experience, a valid Michigan driver's license, and travel throughout Clinton, Eaton, and Ingham Counties.
