Adult Day Services Case Manager

What you'll do

• Assist with eligibility determinations, intake assessments, person-centered care plans, wellness screenings, and client and caregiver resources. Update care plans quarterly
• communicate client conditions with caregivers, families, agency care managers, and physicians
• assist the team with medical emergencies as needed
• support staff training and best-practice recommendations
• complete grant-compliance forms and reports
• and develop internal accountability processes.

Why this role
