What you'll do

• Coordinate interdisciplinary patient care plans from admission to discharge

• recommend and facilitate care plans
• develop discharge plans that address ongoing needs and encourage compliance with medical advice
• identify patient-care issues and suggest clinical pathway improvements
• may conduct utilization reviews.
