What you'll do

• Provide specialized medical case management to individuals living with HIV
• coordinate access to medical care and support services
• assess treatment readiness and adherence
• maintain required documentation and reporting
• enter records into electronic systems
• collaborate with interdisciplinary teams
• conduct rapid HIV testing in office and community settings
• participate in outreach, community events, and patient engagement
• complete required continuing education. Travel throughout the service area is required.
