What you'll do

• Assess residents’ psychosocial, mental, and emotional needs through visitation and observation
• complete social histories and psychosocial assessments
• integrate findings into interdisciplinary MDS care planning
• promote and protect resident rights, dignity, and self-determination
• address abuse, neglect, and exploitation as required
• coordinate discharge planning and post-discharge resources
• provide emotional support and counseling to residents and families.
