The Home Health Registered Nurse (RN) role is an onsite, per-diem position based in Chandler, AZ, serving patients across East Valley communities. Pay is $48.00–$55.00 per hour based on experience. The role provides one-on-one skilled nursing care in patients’ homes with support from a local clinical and leadership team. The nurse assesses patients, develops and updates individualized care plans, delivers nursing interventions, educates patients and caregivers, monitors changes, documents care, and coordinates with the interdisciplinary team. The work supports patient goals, continuity of care, and recovery and stability at home.

What you'll do

• Perform comprehensive patient assessments and contribute to care planning
• develop and update individualized plans aligned with physician orders and patient goals
• deliver skilled nursing services in patients’ homes
• educate patients and caregivers
• provide preventive, restorative, and rehabilitative interventions
• monitor progress and changes in condition
• maintain clinical documentation
• collaborate with the interdisciplinary team.
