What you'll do

• Lead and supervise clinical coordinators, field nurses, and caregiving staff
• oversee and update client Plans of Care
• conduct assessments, reassessments, supervisory visits, and safety evaluations
• review charts and documentation
• manage incident reports and risk mitigation
• support regulatory and Joint Commission readiness
• coordinate staff orientation and training
• coordinate care with providers and families
• support referrals, admissions, discharges, and transitions
• provide skilled nursing care within the Ohio Nurse Practice Act.

Why this role

Current unrestricted Ohio RN license, bachelor’s degree in nursing, 2+ years of home-care nursing, case-management and supervisory experience, CPR certification, and participation in an on-call rotation required.
