What you'll do
- Assess patients and families and determine home-care needs
- reevaluate nursing needs
- initiate and revise plans of care
- develop nursing diagnoses and care plans with patient and family input
- provide preventive and rehabilitative nursing procedures and prescribed medications and treatments
- counsel and instruct patients and families
- identify and implement discharge-planning needs
- prepare clinical notes
- report changes and coordinate with physicians, the agency, and community health contacts
- ensure necessary equipment and services are arranged
- instruct, supervise, and evaluate home health aides every two weeks.
