The Hickory Way Support Coordinator role is based onsite in Ann Arbor, MI. Starting pay is $47,590 per year. The position supports tenants at Avalon Housing’s Hickory Way property. The coordinator connects tenants with services that address housing, mental, physical, and psychosocial needs. Through outreach, case management, crisis support, life-skills assistance, and community-resource connections, the role helps tenants maintain housing and work toward their goals.

What you'll do

• Provide direct case management or coordinate with community case managers
• conduct tenant assessments and develop action plans
• coordinate eviction prevention with Property Management
• provide crisis intervention as needed
• connect tenants with medical, mental health, substance-use, and psychosocial supports
• assist with life skills and budgeting
• link tenants to employment and community resources
• support peer and community programming
• assist with daily living and transportation needs
• maintain clinical records and progress notes.
