Transition Support Coordinator

What you'll do

• Provide face-to-face and/or virtual specialized transition services
• collaborate with care management teams, healthcare providers, and ancillary service providers to implement care-plan support
• document transition activity results and member status
• update care plans as needed
• provide ongoing education
• communicate with healthcare providers as appropriate
• perform other assigned duties.
