What you'll do

• Review and prioritize value-based care reports, care gaps, risk patients, and hospital discharges
• identify and track patients needing preventive and chronic disease care
• contact patients to schedule visits, screenings, and follow-ups
• track discharges and ER visits
• conduct post-discharge outreach within required timelines
• schedule transitional care visits within 7–14 days
• support medication reconciliation and discharge instructions
• coordinate referrals and services
• review charts and document interactions
• use health plan portals and internal systems to track metrics
• assist with audits and quality improvement reporting.
