What you'll do

• Interview patients and family members about home environment, relationships, health history, and psychosocial needs
• assess needs in relation to treatment plans and available resources
• help patients and families understand and follow discharge plans
• participate in care planning conferences and interdisciplinary discussions
• coordinate discharge and post-hospital care
• connect patients with community resources and social service agencies
• collaborate with hospital departments and community partners
• maintain timely, accurate, confidential case documentation
• attend required skilled care and leadership meetings.
