What you'll do

• Manage an assigned caseload of adult and pediatric members, up to 50
• conduct monthly in-person visits
• assess physical, emotional, and social needs
• develop and update individualized care plans
• coordinate with providers and care-team members
• monitor progress and communicate changes
• document interactions and comply with standards
• educate members and families
• connect members with community resources
• coordinate discharge transitions
• participate in training new employees.
