Part-Time Hospice Registered Nurse Case Manager (PRN)

The Part-Time Hospice Registered Nurse Case Manager role is field-based in the Dallas–Fort Worth area, with local travel required. Millenia Hospice provides patient-centered end-of-life care and support for patients and families. The role participates in an on-call rotation as needed. The work supports comfort, dignity, and quality of life in collaboration with patients, families, physicians, and the interdisciplinary hospice team.

What you'll do

• Conduct initial and ongoing nursing assessments
• develop and update individualized plans of care
• provide skilled nursing and symptom management
• coordinate services with physicians and interdisciplinary team members
• educate and support patients and caregivers
• complete timely clinical documentation
• participate in interdisciplinary meetings and the on-call rotation as assigned.
