What you'll do

• Develop nephrology nutrition knowledge for clinically complex dialysis patients in skilled nursing facilities
• maintain patient-specific nutrition information in the EHR
• develop nutrition prescriptions with facility dietary teams
• assess nutrition using lab results and adjust goals and recommendations
• work with patients and the interdisciplinary team to document and update care plans
• educate patients, families, and facility staff on dietary modifications
• participate in quality improvement, IDT and QAPI activities, and continuing education.
