Care Coordinator

Depending on program needs, the work may focus on intake, discharge and aftercare planning, or a combination of both.

What you'll do

• Coordinate intake assessments, documentation, and handoffs as assigned
• support discharge planning, referrals, appointments, and follow-up care
• coordinate services for assigned patients and monitor engagement and care milestones
• participate in treatment-team discussions
• collaborate with Utilization Review
• maintain accurate EMR documentation and follow confidentiality, regulatory, and organizational requirements.
