Care Navigator

What you'll do

• Conduct intake assessments and determine Enhanced Care Management eligibility
• assist with Medi-Cal and other program eligibility
• connect clients to housing, food, transportation, employment, health education, primary care, and behavioral health resources
• coordinate care across providers
• develop and monitor personalized care plans
• schedule and accompany clients to appointments as needed
• conduct outreach and field visits
• document services and referrals
• protect client privacy and follow Medi-Cal guidelines.
