Asian Health Services is developing SpringLight Health, a Program of All-Inclusive Care for the Elderly, to provide culturally competent, coordinated care and support for seniors. The assistant provides nursing interventions under clinical supervision, triages participant needs, and coordinates care and referrals. The role also supports medication administration, documentation, participant education, care transitions, and the interdisciplinary team and quality improvement activities.
What you'll do
- Under supervision, provide nursing interventions and triage participant needs in the clinic or community
- escalate concerns to providers and nursing leadership
- schedule clinic and specialist appointments
- enter appointment instructions in the EHR
- maintain PACE EHR documentation
- administer medications as directed
- disseminate nursing orders and instructions
- coordinate care transitions and communicate with facility case managers
- monitor health and psychosocial changes and report them to the interdisciplinary team
- participate in on-call rotation for after-hours and weekend coverage
- support quality improvement activities.
