Peer Support Specialist — Recovery Coach

What you'll do

• Support clients’ recovery readiness
• provide information and advocacy
• coordinate with the integrated care team
• facilitate warm handoffs to detoxification and inpatient programs and return to outpatient care
• conduct outreach and follow up after discharge
• facilitate peer recovery groups
• connect clients with community services
• provide transportation as needed
• co-facilitate psychosocial rehabilitation groups
• document services and participate in supervision and quality improvement.
