What you'll do

• Develop and maintain referral relationships
• execute weekly territory plans and conduct scheduled face-to-face sales calls
• develop referral sources and strengthen partnerships
• represent the organization at events and health fairs
• coordinate educational presentations
• identify patients appropriate for home health and hospice
• collaborate with clinical teams on admissions
• monitor referral pipelines and follow up
• serve as primary contact for assigned accounts
• promote services and coordinate community marketing activities.
