What you'll do

• Conduct intake interviews and needs assessments
• provide information, referrals, and follow-up
• coordinate with government agencies and civilian providers
• conduct home visits as needed or requested
• maintain resource catalogs and family records
• coordinate monthly support groups or workshops
• present EFMP briefings
• complete monthly reports and facilitate warm handoffs.
