What you'll do

• Participate in developing, monitoring, evaluating, and revising individual program plans
• identify and coordinate services for individuals and families
• complete forms and reports in accordance with regulations and agency policies
• advocate for individuals served by community agencies
• schedule and attend interdisciplinary meetings
• identify service-delivery barriers and use culturally sensitive strategies
• rotate officer-of-the-day duties, provide case coverage as needed, and provide emergency on-call services when required.
