What you'll do

• Provide skilled nursing care to hospice patients
• perform comprehensive assessments and ongoing evaluations
• develop and implement individualized care plans
• provide symptom management and comfort-focused care
• educate patients and families about medications, disease progression, and hospice services
• collaborate with physicians, caregivers, and interdisciplinary team members
• maintain timely clinical documentation

• advocate for comfort, dignity, and quality of life.
