The Registered Nurse (RN), Home Health – Senior Living Facilities role is an onsite, field-based position in Naples, FL, serving assigned assisted living, independent living, and residential care facilities. What you'll do

• Perform nursing assessments and admissions
• develop and manage individualized plans of care with physicians and the care team
• provide skilled nursing visits
• monitor patients and report changes
• educate patients, families, and caregivers
• complete OASIS assessments and documentation
• coordinate with therapists, social workers, physicians, and facility staff
• maintain compliance with Medicare, CMS, and ACHC standards
• participate in case conferences and interdisciplinary meetings.
