Family Support Worker

What you'll do

• Conduct home visits and assess family needs
• provide family-centered, strength-based support to English- and Spanish-speaking families
• deliver curriculum-based prenatal and early-childhood health and development education
• identify crises or additional needs and refer families to community resources
• coordinate meetings in person, by phone, or by video
• document family functioning and report data by program deadlines.
